Step 1:  All YVA students MUST complete the following placement/supervisor data form by the 4th day you meet for YVA class. This will be part of your grade!

Step 2:  All School Day YVA students must read and agree to the YVA Expectations and Requirements by the 4th day you meet for YVA class.  This will be part of your grade!

Step 3:  EACH YVA PERIOD you will need to select the correct form below to log the time you work for your YVA position.  After clicking on the correct form, you will need to complete the top part of the form and have your supervisor fill out the bottom part, digitally sign the form and submit it.  REMEMBER!  It is your responsibility to see that this form gets filled out each day and ONLY your supervisor can "submit" the form after they sign it.

Please make sure you select the right class period!  This is very important so your points can be recorded.

Please review the expectations below.  All absences MUST be made up by your final exam period or you will not receive credit for your YVA class.

Please also make sure your supervisor has completed their evaluations so you don't get a zero on those.

It is YOUR responsibility to get a list of absences from the attendance office so you know how many hours need to be made up.
